Primary Insurance

MIAA Secondary Insurance

All medical bills incurred due to athletic injury/accident must first be submitted to the student-athlete’s general health insurance company.

If a student-athlete does not have general health insurance the coach must obtain a letter from the parent/guardian  verifying "No Primary Insurance Coverage." Please fax a copy of this letter to the MIAA Office.

 

 

A student-athlete injured during official interscholastic play/practice is covered by a secondary MIAA insurance policy. The policy will cover medical expenses not covered by the student-athlete’s primary insurance company up to $10,000 or policy limit. The MIAA secondary policy is administered by Bollinger Insurance.

All student-athletes who have submitted a complete Athlete's Health Record (consisting of Medical Clearance, Release of Claims/Informed Consent and Emergency Information documentation) and included on a current TSSAA Eligibility Report are covered by this policy. This includes all middle and senior high school student-athletes.

All insurance must be paid with a school check. Checks should be made out to MIAA Athletic Health Services. It is requested that the check list the sports team, the number of athletes on the team roster and the total amount paid for that team.  Each sports team payment should be accompanied by an updated TSSAA Eligibility  Report.
