Mentoring Forms
You can find both required and suggested
forms on this page. Required forms are to be completed as directed and
submitted to the school mentor coordinator who will forward them to
the appropriate teacher induction staff development coordinator. Suggested
forms can be used at a school mentoring team's discretion or at the
discretion of individual mentors or proteges.
Required Mentoring Forms
School Mentor Coordinator Commitment Letter --
The mentor coordinator commitment letter outlines the specific responsibilities of the coordinator in the mentoring program. The letter should be signed by the mentor coordinator and school principal in the fall and then submitted to the appropriate teacher induction coordinator.
Protege Needs Assessment
--This document is intended to gather information about beginning teachers'
experience and needs to use in designing appropriate mentor and induction
activities. The assessments should be administered in the fall before
school starts. The completed assessments should be reviewed by the school
mentor coordinator and then submitted to the appropriate teacher induction
coordinator.
Protege Commitment Letter
--The commitment letter outlines the specific responsibilities of proteges
in the mentoring program. The letter should be signed by the protege,
school mentor coordinator, and school principal in the fall and then
submitted to the appropriate teacher induction coordinator.
Mentor Commitment Letter
--The commitment letter and attached position description from the Department
of Human Resources outline the specific responsibilities of mentors.
The letter should be signed by the mentor, school mentor coordinator,
and school principal in the fall and then submitted to the appropriate
teacher induction coordinator.
Mentor Needs Assessment -- This document is intended to gather information about mentoring teachers' experience and needs to use in designing appropriate new teacher support activities for mentors. The assessments should be administered in the fall before school starts. The completed assessments should then be submitted to the appropriate professional development coordinator.
New Teacher Change of Status
Form --This form is used to notify district teacher induction coordinators
when a beginning teacher leaves the school staff for any reason. Completed
forms should be submitted to the appropriate district teacher induction
coordinator.
Suggested Forms
Mentoring Action Plan
--Individual mentors could use this form to plan mentoring activities
and to set a timeline for activites. The form also has space for resources
needed, contacts, and reflections.
School Mentor Team Plan
--Each school's mentor team may wish to complete a plan for teacher
induction and mentoring at the school level each fall. The plan should
be developed under the leadership of the school mentor coordinator and
approved by the principal. District teacher induction coordinators can
assist schools with forming a plan and with using this template to formalize
plans.
Mentor Record of Activities and
Mentor Core Team Documentation Forms --The Mentor Record of Activities
is a template for possible mentor activities. Its purpose is to assure
development of a quality growth relationship between the mentor and
the novice teacher, but also the development of the mentor team in the
school. In most schools, both levels of mentoring are needed. This form
will give a journal-type record of what actually occurred between the
mentor and novice during the year. This form also includes reminders
about mentor core team activities.
