Transcript Requests
For employers who need to verify a graduation date: Fax a copy of the Transcript Request Form with graduate's complete name at time of graduation, year of graduation, and date of birth to the attention of Mrs. Eason.
For alumni who need a copy of a transcript: mail or bring a copy of the Transcript Request Form to the attention of Mrs. Eason along with a $2.00 fee (cash or money order--no checks).

**Note: The BOE requires two forms of ID one should be a picture ID of the person the company is requesting info on. The ID information along with a release of infomation form and/or Transcript Request form should be mailed to the school along with the last year the person attended the school and if this was the last MCS the person attended. The info must be readable or no information will be released. NO FAXES!

Withdrawing Students
Withdrawing Students can be completed from
8:00 -11:30 A.M.
Mon-Fri in the guidance office. Please see Mrs. Eason.
Procedure:

Receive your Withdrawl form
Turn in books and receive withdrawl grade from teachers
If you owe for a textbook or have any other school debts, they must be paid to the school. |

ALL FAMILIES, new and returning, must bring to registration Original Documents ,
Dated in the Past 30 Days, one of the following documents
from each list of approved residential proofs
IN THE NAME OF THE CUSTODIAL PARENT:

Lease agreement
Purchase contract
Mortgage statement
Deed or property tax statement |
Utility bill (MLGW, cable, elephone)

Automobile insurance papers
Auto registration
Health insurance papers
Payroll check with address

Students new to Memphis City Schools are asked to present:
- Birth certificate AND Social Security card
- Immunization record & Evidence of medical exam
- Final report card or withdrawal papers from School Year 2006-2007
- Two proofs of address in CHS’ district (one from each list above)
Students returning to Carver High AND coming from other Memphis City Schools
are asked to present:
- Two proofs of address in CHS’ district (one from each list above)
- Final report card from MCS school last attended
- Child’s Social Security card
